1. Clear Aligner Mechanics
Invisalign is a proprietary orthodontic system that utilizes a sequence of custom-made, transparent thermoplastic aligners to incrementally reposition teeth. The treatment begins with an advanced digital impression of the patient's dentition using an intraoral 3D scanner (such as iTero).
Specialized ClinCheck software is then used to map out the entire biomechanical movement of the teeth from their current position to the target alignment. The dentist places tiny, tooth-colored composite attachments (buttons) onto specific teeth. These attachments act as handles, allowing the aligner plastic to exert precise directional forces. Each set of aligners is worn for 7 to 10 days before transitioning to the next set in the series.
2. Invisalign Package Tiers
Invisalign treatments are divided into distinct packages based on the complexity of tooth movement required, which dictates the number of aligners needed:
- Invisalign Lite: Intended for mild crowding, minor spacing, or orthodontic relapse cases. This package is capped at 14 aligners and typically takes 6 to 9 months to complete.
- Invisalign Moderate: Designed for moderate crowding or spacing issues, providing up to 26 aligners with a treatment duration of 9 to 15 months.
- Invisalign Comprehensive (Full): For severe crowding, complex bite discrepancies, and cases requiring significant rotational or vertical movement. It provides an unlimited number of aligners and refinements over a 3-year window to ensure clinical goals are achieved.
3. Clinical Suitability Cases
While clear aligners have advanced significantly, they are not suitable for all malocclusions:
Suitable Cases: Mild-to-moderate crowding, teeth spacing, simple overbites, crossbites, and resolving alignment issues where bone and gum health is fully stable.
Unsuitable Cases: Severe jaw skeletal discrepancies (which require surgical orthodontics), teeth rotated more than 20 degrees, severe vertical intrusion or extrusion, and cases where teeth must be moved bodily over large distances. Traditional metal braces remain the gold standard for these complex movements.
4. Patient Compliance & Protocol
Because Invisalign aligners are removable, the success of the treatment is entirely dependent on patient compliance. Aligners must be worn for **a minimum of 22 hours per day**. They should only be removed for eating, drinking hot or sugary liquids, brushing, and flossing.
Failing to meet the 22-hour daily wear requirement prevents the teeth from tracking properly according to the digital ClinCheck model. This results in the aligners not fitting correctly, requiring the dentist to take new scans and order "refinement" aligners, which extends the overall treatment duration.
5. Invisalign vs. Metal Braces
When deciding between aligners and brackets, patients should weigh the clinical and lifestyle differences:
- Aesthetics: Invisalign is virtually invisible, whereas metal brackets are highly visible.
- Oral Hygiene: Aligners are removed for brushing and flossing, allowing easy access. Braces have permanent brackets and wires that trap plaque, requiring specialized brushes and floss threaders.
- Dietary Restrictions: Braces require avoiding hard, sticky, or crunchy foods to prevent breaking brackets. Invisalign has zero dietary restrictions since aligners are removed while eating.
- Bite Force: Traditional braces utilize continuous tension, which is highly efficient for closing large gaps and correcting complex bites.

6. 2026 Costs & Funding Policy
Invisalign costs reflect the laboratory manufacturing fees for custom clear aligners (Align Technology) and clinical tracking.
| Invisalign Lite | S$3,500 – S$4,800 |
| Invisalign Comprehensive | S$6,800 – S$9,000 |
| CHAS & Medisave Eligibility | NOT Covered (Elective/Cosmetic status) |
As clear aligner treatments are classified as elective cosmetic procedures, they are not eligible for CHAS subsidies and **cannot be paid via Medisave**. Patients must fund the treatment privately. Most clinics structure Invisalign fees as an initial digital scan fee, followed by a payment upon aligner delivery, and subsequent interest-free installments during check-ups.
